Key Issues:
? The existing box culvert on Stuart Road, just west of the project limits, has deteriorated to the point Stuart Road has been reduced to one lane over the structure.
? Stuart Road is currently closed for the reconstruction of the Stuart Road/Jefferson Street intersection. The onsite contractor has the capacity to replace the deteriorated box culvert on Stuart Road while the road is closed.

Proposed City Council Motion:
FIRST MOTION: I move for a second reading of AN ORDINANCE APPROVING CHANGE ORDER #4 TO THE CONTRACT WITH MIDWEST HEAVY CONSTRUCTION FOR THE JEFFERSON STREET IMPROVEMENTS PROJECT, AN INCREASE OF $126,227.00 FOR A REVISED CONTRACT PRICE OF $6,280,163.23.
SECOND MOTION: I move for adoption of AN ORDINANCE APPROVING CHANGE ORDER #4 TO THE CONTRACT WITH MIDWEST HEAVY CONSTRUCTION FOR THE JEFFERSON STREET IMPROVEMENTS PROJECT, AN INCREASE OF $126,227.00 FOR A REVISED CONTRACT PRICE OF $6,280,163.23.

Background:
During the development of the Jefferson Street project, replacement of the Stuart Road culvert was not considered. While the structure was showing signs of deterioration, it was believed Public Works Operations could repair the structure. As the Jefferson Street project went on, however, the deterioration of the Stuart Road culvert began to accelerate. This accelerated deterioration lead to the failure of the north headwall and portions of the north timber retaining wall. As a result, Public Works Operations was not able to rehabilitate the structure and was forced to close one lane of Stuart Road across the structure.
