Issue/Request:
An Ordinance approving Change Order No. 5 to the contract with Phillips Hardy, Inc. for the Landfill Closure Construction project, an increase of $143,000.00 and 31 calendar days to reach substantial completion for a revised contract price of $3,788,417.83.
Key Issues:
• Repair of erosion on interior and exterior of seep pond dam is necessary to contain landfill seep downstream of the interceptor trench.
• Due to elevations on north side of landfill, the perimeter channel could not extend to adequately capture the flow from the north slope of the landfill. As a result, two terraces were constructed slightly uphill on the north slope to capture storm water and divert it into the perimeter channel.
• Having an all-weather access road to the top of the landfill was needed for long term maintenance of the landfill.

FIRST MOTION: I move for a second reading of an Ordinance approving Change Order No. 5 to the contract with Phillips Hardy, Inc. for the Landfill Closure Construction project, an increase of $143,000.00 and 31 calendar days to reach substantial completion for a revised contract price of $3,788,417.83.
SECOND MOTION: I move for adoption of an Ordinance approving Change Order No. 5 to the contract with Phillips Hardy, Inc. for the Landfill Closure Construction project, an increase of $143,000.00 and 31 calendar days to reach substantial completion for a revised contract price of $3,788,417.83.
Background:
The construction of final cover over an approximately 82-acre landfill to include excavation of soil materials from borrow areas, placement of various soil layers on landfill surface, installation of geosynthetic components, erosion controls at landfill and in borrow areas, and establishment of vegetation in disturbed areas as well as all labor, materials, equipment, tools, superintendence, and any and all other services necessary to perform the work

Recommendation
Staff recommends approval of an Ordinance approving Change Order No. 5 to the contract with Phillips Hardy, Inc. for the Landfill Closure Construction project, an increase of $143,000.00 and 31 calendar days to reach substantial completion for a revised contract price of $3,788,417.83.
Committee Recommendation
The Public Works Committee voted unanimously to recommend to City Council approval of an Ordinance approving Change Order No. 5 to the contract with Phillips Hardy, Inc. for the Landfill Closure Construction project, an increase of $143,000.00 and 31 calendar days to reach substantial completion for a revised contract price of $3,788,417.83.
